After Care Coordinator – LifeShare Carolinas

Essential Functions

  • Supports grieving families and professional staff.
  • Provides access to community resources for donor families such as grief counseling and support groups.
  • Develops and manages bereavement workshops and events for donor families.
  • Ensures appropriate follow up with donor families to include evaluating and facilitating a response to post-donation requests from donor families.
  • Works with Community Services to lead volunteer team to expand outreach to community.
  • Participates in quality assurance and performance improvement.
  • Supports and engages staff in the connection to the mission of LifeShare.

Education, Experience and Certifications

Bachelor's Degree in counseling, social work, chaplain or other related field required. Master's Degree preferred. Valid driver's license required. Experience with organ, tissue, and eye donation and transplantation preferred. Experience in the fields of trauma, grief, and bereavement preferred.
