After Care Coordinator – LifeShare Carolinas
Where You Will Work
LifeShare Carolinas coordinates the recovery and distribution of organs, eyes, and tissues for transplantation and medical research across Western North Carolina. Based in Charlotte, we facilitate donation in 25 counties. Additionally, LifeShare educates the public about donation and provides support for the families of our organ, eye, and tissue donors during and after donation.
Essential Functions

Responsible for coordinating the post-donation activities with donor families to ensure compliance with Association of Organ Procurement Organizations (AOPO) and Centers for Medicare and Medicaid Services (CMS) requirements.

Responsible for developing and implementing a grief support program for donor families.

Supports grieving families and professional staff.

Provides access to community resources for donor families such as grief counseling and support groups.

Develops and manages bereavement workshops and events for donor families.

Ensures appropriate follow up with donor families to include evaluating and facilitating a response to post-donation requests from donor families.

Works with Community Services to lead volunteer team to expand outreach to community.

Participates in quality assurance and performance improvement.

Supports and engages staff in the connection to the mission of LifeShare.
What You Will Need:

Bachelor's degree in counseling, social work or chaplain required

Master's Degree preferred.

Experience with organ, tissue, and eye donation and transplantation preferred.

Experience in the fields of trauma, grief, and bereavement preferred.

Valid driver's license required.
Physical Requirements
- Ability to sit, stand, bend and walk.
- Lift and pull at least 30 pounds.
- Ability to be contacted by phone or pager.
- Ability to work outside of normal business hours and travel.
- Ability to use personal vehicle for business
