Home Health Occupational Therapist (OT)
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
No weekends or holidays
Coverage Area: Chicago ZIP codes 60608, 60609, 60615, 60616, 60623, 60653

Major Responsibilities
Quality of Care
- Deliver high-quality, cost-effective care aligned with evidence-based practice
- Utilize appropriate assessments, interventions, and outcome measures
- Identify and address risks to patient safety and outcomes
- Maintain thorough, accurate documentation to support regulatory and reimbursement requirements
- Participate in quality and process improvement initiatives
Care Coordination
- Collaborate with the interdisciplinary team to develop and adjust care plans
- Recognize changes in patient condition and modify treatment accordingly
- Coordinate referrals, discharge planning, and ongoing care needs
- Participate in case conferences and ensure effective communication using SBAR and other tools
Patient-Centered Care
- Develop individualized care plans addressing physical, psychosocial, and functional needs
- Educate patients and families to promote independence and self-management
- Adapt communication style to meet patient needs and promote engagement
- Deliver compassionate care that exceeds patient and family expectations
Leadership & Collaboration
- Work effectively with team members across disciplines
- Participate in staff meetings, trainings, and professional development
- Support a collaborative, respectful team environment
Operational Excellence
- Manage time and resources effectively across a home-based caseload
- Adhere to organizational policies and productivity expectations
- Support efficient care delivery and optimal patient outcomes
Education & Experience
- Master’s Degree in Occupational Therapy
- Graduate of an AOTA-accredited program
- Minimum 1 year of clinical experience (or strong clinical internship background)
- Current Illinois OT license (or eligibility)
Licensure & Certifications
- Licensed Occupational Therapist – State of Illinois
- BLS certification (or obtained within 6 months)
- Valid driver’s license
Skills & Qualifications
- Strong clinical assessment and treatment planning skills
- Excellent communication and documentation abilities
- Strong time management and organizational skills
- Comfortable using EMR/documentation systems
- Proficiency with home health therapy equipment and adaptive devices
Work Environment & Physical Requirements
- Travel within assigned geographic territory in varying conditions
- Work in patient homes and assisted living environments
- Ability to lift up to 35 lbs (with assistance/devices as needed)
- Physical activity including bending, lifting, transferring patients, and stair navigation
