Schedule Details/Additional Information:
Mon-Fri, typical clinic hours ~8-5.Position would be hybrid; primarily remote when not on site with clinic teammates. Oversight of ambulatory clinics in WI and N IL.

Major Responsibilities:

  • Manages and oversees daily operations of interdisciplinary chronic disease management teams by establishing priorities, providing work direction, resolving problems and monitoring department performance. Ensures that services are safe and high quality. Ensures appropriate department staff levels and oversees scheduling. Monitors productivity and makes appropriate adjustments. Ensures optimal use of supplies and drugs for cost-effectiveness and efficiency.
  • Supports chronic disease management and population health goals at both the system and site level. Leads the implementation of system initiatives and policies, and monitors compliance at the site. Collaborates with other site leaders to meet site goals and resolve site issues. Identifies quality and safety risks in department operations and clinical practices. Establishes and implements quality and safety improvements. Leads quality improvement projects within the department and site.
  • Oversees planning, implementation, supervision, evaluation, and adherence to national standards for diabetes education programs.
  • Supports and develops chronic disease management procedures within physician based or other clinics, such as implementing appropriate clinical protocols, patient education, collaborative practice agreements, refill protocols, step therapy protocols, drug therapy changes or titration and comprehensive medication management activities. Leads interdisciplinary teams that may include registered dieticians, nurses, pharmacists or other disciplines.
  • Performs human resources responsibilities for staff which include interviewing and selection of new employees, promotions, staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions, terminations, and overall employee morale. Develops and recommends operating and capital budgets and controls expenditures within approved budget objectives.

Licensure:

  • Pharmacist, Registered

Education/Experience Required:

  • Bachelor’s Degree in Pharmacy Typically requires 5 years of experience with at least 1 year in a supervisory role management staff and overseeing budgets. Practice consistent with the position’s roles/responsibilities.

  • Pharmacist license in the state of practice Issued by Stated Board of Pharmacy
