Schedule Details/Additional Information:
Clinic hours: 7-5 Mon, Thurs, Fri; 7-6 Tues, Weds; 7-11 Sat (not open every Saturday); 1.0 FTE = 40 hours per week.

Major Responsibilities:

  • Direct nursing care:
    •     3)Is knowledgeable and able to assist with job duties of other personnel in the department.
    •     1)Prepares patients for exams and treatments physically and mentally and monitors their response.
    •     2)Capable of assisting physician with all procedures and treatments.
    •     4)Consistently encourages questions from patients and their families and offers guidance that fosters compliance.
    •     5)Administers medications according to approved policy/procedure following protocol or physician order and monitors response.
  • Indirect Nursing Care:
    •     1)Consistently orders tests and reviews results according to department protocol or as ordered by physician.
    •     2)Reports abnormal findings, patient complaints, or concerns, to physician or supervisor.
    •     3)Assures the accuracy and completeness of messages. Prioritizes messages, communicating information to providers/patients in a timely manner.
    •     4)Explains procedures and test results using language appropriate to patient’s level of understanding.
    •     5)Serves as an advocate and liaison between patients, staff, physicians and external customers by sharing necessary information to ensure continuity of care.
  • Documentation:
    •     1)Provides consistent and accurate documentation of patient information, care and concerns in the medical record according to the guidelines provided in the Documentation Policy and as otherwise instructed.
    •     2)Consistently completes vouchers, referrals and requisitions with all appropriate information including medical record number and services codes or using the medical record.
    •     3)Completes all patient documentation according to the Missing Documentation policy guidelines.
  • Time Management:
    •     1)Consistently manages time and demonstrates resource management skills effectively to complete daily activities.
    •     2)Delegates tasks to others when appropriate.
    •     3)Manages patient flow by keeping rooms clean, stocked and available and maintains adequate inventory.

Licensure:

  • Nurse, Licensed Practical (LPN)

Education/Experience Required:

  • Graduated from an accredited LPN school.

Knowledge, Skills & Abilities Required:

  • Good communication skills. Basic keyboarding and typing skills.
  • Current LPN license in the state of Illinois. CPR certification required upon hire or within 60 days of hire.

Physicial Requirements and Working Conditions:

  • Must be able to successfully complete clinic-based training for all associates. Ability to work with all levels of personnel. Ability to work under stressful situations. Frequent periods of extended standing and sitting. Possible lifting and assisting of patients.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

Addendum: GENERAL NURSING REQUIREMENTS • Demonstrates comprehension of basic disease processes. • Demonstrates competence in administering IM, subcutaneous and intradermal medications. • Demonstrates knowledge of common maintenance medications and nursing protocol to refill and advise. • Demonstrates competency in suture/staple removal. • Demonstrates ability in interpreting test results and notifies appropriate medical staff of results as necessary. ALLERGY • In addition to the general nursing requirements: • Demonstrates ability to complete allergy skin testing. DERMATOLOGY • In addition to general nursing requirements: • Able to assist with small procedures, biopsies, excisions and giving appropriate wound care and other patient instructions. • Assist with laser procedures especially aftercare. • Knowledge of application of PUVA therapy. ENT • In addition to general nursing requirements: • Able to assist with small procedures, biopsies, wound care and other patient instructions. • Able to schedule all physicians’ surgeries efficiently. • Documented successful completion of ENT competencies. FAMILY PRACTICE • See Internal Medicine and Pediatrics GASTROENTEROLOGY • In addition to general nursing requirements: • Demonstrates ability to cleanse, set up, and test flexible sigmoidoscopy equipment. • Able to provide patient education and accurately schedule for all procedures. GENERAL SURGERY • In addition to general nursing requirements: • Demonstrates ability to provide patient education on wound care, A/V fistula’s, preoperative and postoperative care. • Demonstrates ability to accurately schedule surgical procedures and tests at hospital locations, along with follow up physician appointments related to those procedures. • Able to assist with biopsies, excisions, I & D’s (incision and drainage), needle aspirations, hemorrhoids, una boots, suture/staple removals, dressing changes, and give appropriate patient instructions. INTERNAL MEDICINE • In addition to the general nursing requirements: • Demonstrates competency in the growth and development of the adult patient (18 years +). • Demonstrates ability to monitor respiratory status, competently administers nebulizer treatments and evaluates patient response. • Demonstrates ability to competently perform ear wash. • Demonstrates ability to competently perform EKG’s. • Demonstrates ability to assist provider with paps and provide accurate patient education. NEPHROLOGY • In addition to general nursing requirements: • Demonstrates knowledge of kidney biopsy and transplant procedures. • Ability to accurately maintain procrit and epogen flow sheets. NEUROLOGY • In addition to general nursing requirements: • Demonstrate ability to attend spinal tap and assist physician. • Assist physician with wound debridements. OB/GYN • In addition to general nursing requirements: • Exhibits sterile technique and ability to assist with department specific procedures such as colposcopies, cyrotherapy, circumcisions, and loop electrosurgical excision procedure. • Demonstrates competency in performing fetal monitoring via non stress testing including knowledge of reactive vs. non-reactive strips. • Demonstrates ability to accurately complete new OB history on prenatal flow sheet. • Demonstrates ability to accurately assess Fetal Heart Tones via Doppler. • Demonstrates ability to competently perform urine pregnancy testing and uristix testing for albumin and sugar via lab protocols. OPHTHALMOLOGY • In addition to general nursing requirements: • Able to assist with small procedures, excisions, wound care and other patient instructions. • Demonstrates ability to perform preoperative and postoperative teaching to patients and their significant others. • Demonstrates ability to perform specified procedures according to department guidelines. • Demonstrates the ability to perform the digital photography for the Fluorescein Angiograms. • Demonstrates the ability to appropriately administer dilating drops, glaucoma drops and abx drops/ointments. • Demonstrates the ability to perform A-Scans and take Keratometry readings. • Able to demonstrate the ability to remove stitches under the direction of the physician. • Able to demonstrate the ability to perform Visual fields, Gads, auto-Refractions, Topography and P.A.M.s. ORTHO • In addition to general nursing requirements: • Demonstrates ability to apply and remove casts. Proficient in applying various splints and soft goods. • Exhibits good technique in dressing removal and application including post-op, wet to dry dressings, adaptic, packings, external fixator pin care. • Demonstrates ability to schedule procedures, tests, and simple surgeries in ASC or hospital. • Demonstrates competency in patient teaching related to cast care and application of splints and soft goods. • Demonstrates competency to complete detailed insurance forms required for surgeries and worker’s compensation authorizations for procedures, testing and surgery. • Demonstrates ability to schedule and give patient instructions for complex surgical cases (includes total joints and spine cases.) • Demonstrates ability to set up, draw meds and prep for joint injections. PEDIATRICS • In addition to the general nursing requirements: • Demonstrates knowledge of growth and development of the pediatric patient (neonate to 18 years). • Documented successful completion of pediatric competencies PSYCH • Demonstrates ability to triage crisis situations including suicidal patients. • Monitors and maintains a therapeutic/safe environment according to department guidelines, policy and procedure. • Knowledge in the maintenance of department flow sheets for monitoring labs and other parameters when taking specific medications. • Demonstrates knowledge and legal ramifications of the strict measures for confidentiality that are mandatory in the department. • Demonstrates safe and effective management of the aggressive patients. RHEUMATOLOGY • See Internal Medicine. UROLOGY • In addition to general nursing requirements: • Demonstrates ability to assist with specified procedures according to department guidelines, including but no limited to: a. Male or female exam, with or without dilation b. Change urinary catheter on male or female c. Residual catheterizations d. Marshall test e. Cystoscopy – male or female f. Prostate ultrasounds and biopsies g. Vasectomies h. Excision and cautery of lesions • Demonstrates ability to accurately schedule surgical procedures and tests. • Demonstrates ability to provide patient education.
