Schedule Details/Additional Information:
Cover the Multi Disciplinary clinics Cystic Fibrosis and Neuromuscular MDC Clinic. Will also support the Adult CF one day per month. Also serve as a resource when the Pediatric Subspecialty Social worker is on PTO. Perform Mental Health Assessment for the CF patients.

Major Responsibilities:

  • Provision of direct discharge planning and clinical social work intervention to patients and families, in consultation with the health care team.
    •     1)Screen all patients identified through high risk criteria and through the referral process.
    •     2)Develops discharge plan in direct consultation with the patient, family, physician and health care team; coordinates arrangements to implement plans which includes facilitating referrals for home care, alternative facility placements etc.
  • Collection, interpretation, and assessment of information regarding the patients.
    •     2)Communicates with other members of the healthcare team in order to obtain information needed to complete an individualized discharge plan for the patient.
    •     1)Interviews patient and patient’s family (if appropriate) to assess the patient’s Psychosocial and post hospital needs.
  • Supportive counseling of patients and their families suffering from acute emotional distress (i.e. adoptions, physical disabilities, terminal illness, abuse and trauma).
    •     1)Counsels patients and families regarding issues of loss, disability, separation and traumatic lifestyle changes.
    •     2)Counsels and assists patients and families regarding Advanced Directives and ethical issues that may arise.
    •     3)Interacts with appropriate agencies on issues of suspected child abuse, domestic violence, guardianship and other social/legal matters.
  • Adherence to all department and facility procedures regarding documentation in the medical record, compilation of statistics, and other data collection as well as attendance at staff meetings, inservices and unit patient care conferences.
    •     1)Documents all contacts with the patient, family and health care team in the patients medical record per department policy.
    •     2)Compile statistics on patients in regards to services rendered.
    •     3)Attends department staff meetings, inservices and unit patient care conferences.
    •     4)Promotes patient safety by reporting of issues through established channels and participating as requested in safety initiatives.

Licensure:

  • Social Worker, Clinical

Education/Experience Required:

  • MSW

Knowledge, Skills & Abilities Required:

  • Counseling and psychotherapy skills. Knowledge of aftercare resources.
  • LSW-Illinois (if eligible) Must obtain within one year of employment.

Physicial Requirements and Working Conditions:

Ability to work under stressful conditions and in difficult situations. d. May be exposed to life-threatening diseases. e. *This position needs to recognize needs and behaviors of specific age groups of patients treated.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
