Teen Parent Program Coordinator

General Info:

• Under the supervision of the Health Family Programs Supervisor, the Program Coordinator is responsible  for the the clinical, administrative, and educational activities within the teen parent  programs of Baby Steps and Healthy Families Illinois.  This includes program development, strategic planning, funding development and personnel management of a  multi-disciplinary team in on-site and home-based settings. Scope of authority includes direct supervision of Family Support workers.  Functional areas of responsibility include report preparation, data evaluation, and direct management and budget accountability for the Department of Human Services and private grantors.

Major Responsibilities:

• Provides clinical direction for Baby Steps and Healthy Families Illinois Programs to ensure program effectiveness
  •     1)Oversees assessment of families eligible for teen parent programs.
  •     2)Coordinates referrals and services with OB/Gyne Clinic, Mother-baby nursery, and NICU.
  •     3)Assigns cases to clinical staff for ongoing services.
  •     4)Monitors ongoing treatment plans and services provided by clinical staff assuring compliance with programmatic goals for client participation and behavioral outcomes.
  •     5)Maintains and develops curriculum content for clinic workshops and home visiting sessions.
  •     6)Advocates and facilitates continuity of care for children ad their families within the programs and with appropriate community agencies.
  •     7)Provides planning and program development activities for the teen parent programs, including the development of fund-raising strategies, marketing and new program activities.
• Provides supervision of clinical staff to maximize achievement of treatment goals.
  •     1)Provides initial orientation and training for direct service staff in the programs
  •     2)Meets with clinical staff in weekly clinical supervision.
  •     3)Participates in appropriate Healthy Families Illinois training. Assures all Healthy Families Illinois staff competence in all required core training. Organizes and presents ongoing inservice training activities for Healthy Families and Baby Steps staff.
  •     4)Provides supervision of direct service staff: Healthy Families Assessment Worker and Family Support Workers.
• Provides direct clinical service to children and families
  •     1)Provides psychosocial services to families in both clinic setting and home environment including, but not limited to, individual counseling, crisis intervention, crisis intervention, service coordination, and resource and referral.
  •     2)Conducts group developmental workshops with parents during clinical sessions
  •     3)Assists the clinical director in program development, including preparation of proposals for programmatic support.

Education/Experience Required:

• Master’s Degree in Social Work or related human service field. 2-3 years clinical experience with children and families. 1-2 years supervisory experience.

• Appropriate clinical skills. Good communication skills. Good case management skills. Administration and program development skills. Strong organizational skills

Physical Requirements and Working Conditions:

• May be exposed to adverse, hazardous or unpleasant conditions. Home visiting required. Ability to work in team-oriented environment.
