Title: Nurse Navigator – Structural Heart
Scheduled Hours:
- Monday through Friday
- 8 hour shifts during the day
- Full Time-80 hours a pay period
Job Summary:
As navigator for Structural Heart, RN will work collaboratively with structural heart team, inpatient, outpatient, PCPs, cardiologists and other healthcare providers to ensure effective and efficient throughput of structural heart patients.
Within their responsibilities the Structural Heart (SH) RN Navigator will maintain ongoing database and additional data to manage with structural heart team to improve/expand program. Provides professional expertise in management of SH care across the continuum of care. Collaborates with physicians, nursing staff, and other health care providers to provide services to SH patients and caregivers.
The nurse assists with initial evaluations with focus on physical assessment, education, discharge planning, disease management, ongoing monitoring and early interventions to provide quality cost effective care. May establish relationship with the patient/families after discharge from hospital and work collaboratively with their physician.
Position Requirements:
- Graduate of an accredited school of nursing with Current IL RN license
- BSN required
- Current CPR and ACLS
- Must possess at minimum 3 years of recent acute cardiac care nursing experience
- Certification in your current area of specialty within 2 years of hire
- Strong oral and written communication skills
- Good physical and psychosocial assessment skills
- Ability to be self directed and responsible for work
- Able to problem solve and prioritize efficiently
- Able to collaborate effectively with multidisciplinary team
- Strong teaching and coaching skills related to life style modifications
- Must be mobile to see patients in all hospital units. Some travel to extended care facilities and nursing agencies
- Work environment includes probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick
- Ability to respond to patients in a timely manner
- Flexible hours
- The candidate must be able to perform physical tasks such as lifting patients and moving equipment
- Ability to lift up to 50 pounds occasionally and/or 25 pounds frequently and/or 20 pounds constantly.
