Schedule Details/Additional Information:
Outpatient hematology/oncology LCSW position. 0.5 FTE (16 hours/24 hours per week)
Major Responsibilities:
- Provision of direct discharge planning and clinical social work intervention to patients and families, in consultation with the health care team.
- Collection, interpretation, and assessment of information regarding the patients.
- Supportive counseling of patients and their families suffering from acute emotional distress (i.e. adoptions, physical disabilities, terminal illness, abuse and trauma).
- Adherence to all department and facility procedures regarding documentation in the medical record, compilation of statistics, and other data collection as well as attendance at staff meetings, in-service's and unit patient care conferences.
Licensure:
- Social Worker, Clinical- LCSW
Education/Experience Required:
- MSW
Knowledge, Skills & Abilities Required:
- Counseling and psychotherapy skills. Knowledge of aftercare resources.
- LSW-Illinois (if eligible) Must obtain within one year of employment.
Physicial Requirements and Working Conditions:
- a. Flexible hours according to customer needs. b. Ability to prioritize independently and respond to multiple simultaneous requests. c. Ability to work under stressful conditions and in difficult situations. d. May be exposed to life-threatening diseases. e. *This position needs to recognize needs and behaviors of specific age groups of patients treated.
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
