This role is majority on-site with patient-facing availability, with some remote option.
Major Responsibilities:
Collaborates with the orthopedic team members to assess and improve all aspects of the orthopedic program as indicated by quality assessments, including the development/revision of clinical pathways and/or protocols, based on program data and best practices. Identifies and addresses necessary improvements to patient care, safety, environment, and infection control for the orthopedic patient population.
Responsible for implementing/sustaining a program that meets DNV and BCBS Blue Distinction standards. Responsible for the oversight, maintenance, and compliance of accreditation standards for the orthopedic program at applicable site/sites.
Maintains a Quality Improvement Plan to achieve and maintain the designation of DNV/BCBS accreditation.
Organizes and leads a multi-disciplinary team in monitoring and improving our services to orthopedics patients.
Works collaboratively with staff throughout the hospital to increase competence and confidence in caring for orthopedics patients and reduce the risk of injury to patients and staff. Identifies staff learning needs and provides/coordinates evidence-based learning opportunities that will increase staff's knowledge and improve clinical performance.
Serves as a liaison between physicians, nursing staff, ancillary staff, patients and families to promote patient satisfaction and enhance continuity of care.
Completes a clinical assessment to identify the patient’s individualized needs and supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care, and identifying barriers to care. Navigates patients and family members throughout the continuum of care including diagnosis, pre-procedure, treatment, and follow-up phases.
Serves as a liaison between physicians/physician offices, nursing staff, ancillary staff, patients and families to promote patient satisfaction and enhance continuity of care.
Conducts pre-op classes for pre-surgical patient populations and engages in routine follow-up/monitoring post discharge. Provides education and outreach programs to patients, internal care teams, and members of the community. Evaluates, selects, or designs patient education materials on specific disease site programs.
Rounds on patients and families in all care areas to assess how well their needs have been met and identify opportunities for improvement. Manages patient care access points including intake of patient calls.
Supports service line growth initiatives to meet or exceed budgeted volumes.
Monitors physician referral patterns and partners with leadership to develop programmatic initiatives best suited to patient needs and market trends.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Registered (RN), OR Doctor of Physical Therapy (DPT)
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.

BSN or DPT
Experience Required:
Typically requires 4+ years of experience that includes experience in clinical assessment, care plan development, and patient education across acute and ambulatory care settings.
Knowledge, Skills & Abilities Required: Working knowledge of the components of quality and patient care needs specifically related to orthopedic total joint and trauma patients. Maintains clinical expertise to effectively manage physical, emotional, psychosocial and spiritual needs of patients throughout the care continuum. Excellent analytical and interpersonal communication skills necessary to negotiate with families, patient, physicians, and third party payers. Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
Physical Requirements and Working Conditions:
Clear oral communications and hearing acuity required for receiving instructions and converse on standard telephone. Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone. Exposed to normal office environment.
