Schedule Details/Additional Information:
This position will be responsible for staff at Christ/Children's Oak Lawn and South Suburban.  They will also be responsible for community partner work in the community. 

Major Responsibilities:

  • Provides oversight of the hospital case management process for intentional trauma survivors, ensuring high quality patient care in accordance with established policies, procedures and overall strategic plan and expectations.
  • Provides oversight of the case management and discharge process to ensure appropriate follow up care, connection to resources and safety from retaliation. Ensures that the TOW team facilitates continuity of care-related issues such as application requirements, charity care paperwork, victim compensation, etc.
  • Maintains up-to-date information for the interdisciplinary team regarding resources that are available in the community and the health system for Trauma patients and family.
  • Maintains referral relationships and communication with physicians, program managers, staff, employee assistance programs and clinics building the reputation, overall productivity and grant compliance of Advocate Trauma Recovery Center.
  • Create procedures to collect data for analysis which measure and report the effectiveness of the TOWs in patient care and workplace violence. Presents information to key management, allowing them to make decisions about services provided.
  • Ensures effective and timely communication for the interdisciplinary and multidisciplinary teams, including but not limited to street outreach, Trauma Recovery Center clinical team, nurses, social workers, physicians.
  • Sets goals, objectives and action plans which are aligned with the overall strategic plan of Advocate Trauma Recovery Center. Monitor performance measures which will enhance the quality of services provided and are based on substantiated problem areas.
  • Implements procedures which enhance the ongoing care of patients served. Ensures that department/unit operations are in compliance with all policies, procedures and regulatory agency requirements and department practices. Enforces, writes, develops and monitors policies and procedures which support regulatory requirements.
  • Performs human resources responsibilities for staff which include interviewing and selection of new employees, promotions, staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions, terminations, and overall employee morale.
  • Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations and laws applicable to the organization's business.
  • Responsible for scheduling and coverage across multiple hospitals being served by the Advocate Trauma Recovery Center.

Licensure, Registration, and/or Certification Required:

  • None Required.

Education Required:

  • High School diploma or equivalent

Experience Required:

  • 3+ years of management experience
  • 3+ years of social service experience

Knowledge, Skills & Abilities Required:

  • Thorough understanding of the issues of importance to referral sources.
  • Demonstrates knowledge of mental health code, grant requirements, federal and state regulations and ability to train staff in these areas.
  • Requires a systems perspective for problem solving, decision making, communication, quality improvement and customer service.
  • Understands the criteria for levels of care and the clinical rationale for utilization of the continuum of care.
  • Excellent human relations skills, analytical skills and communication skills.
