Major Responsibilities:
Provides direct patient care within scope of practice
1) Coordinates ongoing patient care and planning of appropriate patient services (e.g. scheduling appointments, counseling sessions, consultations, arranging referrals, and admissions to the hospital). Functions as a liaison and patient advocate.

2) Utilizes the nursing process and physical assessment skills to provide in-person nursing care, such as walk-in assessment, chronic care management, and health screenings

3) Assesses, plans, delivers, and evaluates care for each patient based on developmental age of the patient

4) Administers medications as ordered by the physician and in accordance with nursing protocols and standards of practice

5) Provides patient education and counseling for patients in one-on-one or small group setting

6) Demonstrates competency and performs in-house clinical laboratory exams (e.g. urine dipstick, glucose test).

7) Works collaboratively with other hospital departments and other health care providers, to facilitate scheduling and medical work-up of patients

8) Assists Residents/Attending physicians and staff with procedures and examinations in accordance with department guidelines

9) Assures that patient charting and clinical documentation are complete, including patient consent forms, encounter forms, etc.

10) Reviews various test results and reports, records information, and notifies physicians appropriately. • Identifies the need for and initiates emergency patient care. Performs CPR as necessary. • Participates in the maintenance of appropriate supply inventory and proper functioning of department equipment. • Maintains a safe, comfortable, and confidential environment for patients at all times. • Performs Antenatal tests per department guidelines. Informs attending physician of abnormal test results. Informs referring provider of test results as appropriate. • Performs other duties as assigned. •

Provides professional telephone nursing advice and follow-up
1)Works with designated Resident/Attending physician to provide telephone assessment and home therapies consistent with established guideline

2)Performs telephone triage, making assessments regarding level of care needed (i.e. emergent care, office visit, self-care), consulting with providers as needed

3) Conducts patient follow-up by telephone when professional nursing expertise is required

4) Documents ongoing assessments of the patient’s physical and emotional status

5) Phones in prescriptions and refills medications, per physician orders, and consistent with established center guidelines for medication management.

3) Knowledge of and compliance with federal, state, and local laws that apply to the job’s area of responsibility

4) Functions as a role model and resource for clinical and non-clinical support staff, Resident physicians, and students.

5) Assists in orientation of new staff and students

6) Displays problem-solving and decision-making, decision sharing and conflict resolution skills to support center activities

7) Participates in the development of educational tools and protocols

8) Promotes continuous improvement in the effectiveness of the practice, by participating in QA/QI activities

9) Participates in committees as requested

10) Adheres to flexible work schedule.

2) Maintains current Illinois nursing license

3) Maintains active Healthcare Provider in CPR status

4) Maintains certifications as appropriate.

5) Attends meetings , inservices and Advocate sponsored programs

6) Completes annual mandatory education activities and all appropriate clinical competency assessments

Licensure:
Nurse, Registered (RN)

Education/Experience Required:
Effective January 1, 2012: • Bachelor of Science in Nursing (BSN) Education requirememnt if hired prior to 01/01/2012: • Graduate of an accredited school of nursing, BSN preferred. • Minimum of one year nursing experience; ambulatory care experience preferred. • Board Certification by 12/31/2015 or within two years of hire

Knowledge, Skills & Abilities Required:
Clinically appropriate skills. Strong ability to communicate clearly effectively. Strong interpersonal skills and an awareness of customer relations. Strong time management and organizational skills. Ability to work independently. Familiarity with computers.
Current RN license in the state of Illinois.

Physical Requirements and Working Conditions:
Ability to work varying shifts to meet the needs of the center. Ability to work under stressful conditions and in difficult situations. Ability to perform lifting/transfer activities related to patients as needed. Ability to stoop/bend. Ability to handle multiple demands. Routinely works with potentially infectious specimens and patients. May be exposed to hazardous materials and life threatening diseases.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
