Pre -Transplant Clinical Coordinator (RN)

Position Overview

We are seeking an experienced (RN) to join our multidisciplinary transplant team. This role is critical in coordinating the evaluation and care of transplant candidates and recipients, ensuring seamless communication among patients, families, and healthcare providers. The ideal candidate will have a strong clinical background, excellent organizational skills, and a passion for patient-centered care.

Key Responsibilities

  • Collaborate with surgeons, nephrologists, hepatologists, transplant cardiologists/pulmonologists, and the Transplant Selection Committee to coordinate evaluations for transplant candidates.

  • Arrange diagnostic testing, identify clinical issues, and analyze pertinent data.

  • Provide education and counseling to patients and families; address barriers to postoperative compliance and recovery.

  • Assist physicians during clinic visits, coordinate consultations, and communicate with home care agencies and insurance providers.

  • Monitor immunosuppression protocols and patient progress; ensure continuity of care post-transplant.

  • Maintain accurate electronic medical records and submit required data to transplant registries.

  • Ensure compliance with regulatory documentation (UNOS, Medicare).

  • Participate in physician outreach, community engagement, and Living Kidney Donor Program growth initiatives.

  • Coordinate volunteer activities and assist with fundraising/community events.

  • Perform additional duties and special projects as assigned.

Required Qualifications

  • Licensure:

    • Registered Nurse (RN) license in the state of practice.

    • Clinical Transplant Coordinator (CCTC) certification issued by (ABTC) within 2 years of hire

    • Basic Life Support (BLS) certification within 6 months (AHA).

  • Education:

    • Associate’s Degree in Nursing (BSN preferred).

  • Experience:

    • Minimum 5 years of cardiovascular and/or clinical practice experience, including transplant, end-stage cardiac disease, and/or end-stage liver or renal disease.

Skills & Abilities

  • Strong interpersonal and communication skills.

  • Proficiency in Microsoft Office Suite.

  • Ability to work independently, manage multiple priorities, and meet deadlines.
