Hospice

Home Health

You’ll find that patients love being cared for in the comfort of their own homes. As a home health team member, you’ll work at the top of your license, benefit from the unique nature of one-to-one care delivery and enjoy autonomy as you care for your patients. You’ll be championed by your leaders and supported by a team that includes administrative professionals. Your patients may be recovering from illness, injury or surgery or managing a chronic disease or complex condition. Registered Nurses and Case Managers

Home Health

As a home health nurse or case manager, you will use your skills and expertise to care for patients with a wide range of chronic and complex conditions. You will work closely with your leaders and the other members of the home health team to implement your patient’s care plan. Teaching is key, as you will be responsible for educating the patient and their caregivers about their conditions, safety issues, and factors affecting the road to recovery.

Hospice

Your work as a hospice nurse will have a profound impact on patients and families facing the end of life. You will use your nursing skills and expertise to care for your patients during a very challenging time. You will also serve as the “hub” of the hospice team, connecting the care providers and services needed to ensure that your patients get all the physical, emotional and spiritual support they need and that their loved ones can confidently care for them when you aren’t there.

Licensed Practical Nurses (LPN)

As a home health or hospice LPN, you will be an important member of the nursing team. You’ll work closely with RNs to help support our patients and families, providing care that may include symptom management or helping patients who experience a medical crisis.

Aides

As a home health aide, you will help patients in a variety of ways, including providing personal care such as bathing or dressing, along with other activities of daily living. Under the nurse’s direction, you may also take patients’ vital signs or give them medicine. You’re an important part of the team, working closely with the RNs and letting them know of any changes that need their attention.

Social Workers

Social work services are available to all of our home health and hospice patients and their families. As a member of this team, you’ll assess the patient to determine whether emotional, social or practical support is needed. You’ll ensure that patients and families have the services and support they need, often connecting with community resources. In home health, your work will also include assisting patients in treatment planning and participating in the formulation of discharge plans. And in hospice, you’ll counsel patients and their loved ones, bringing members of the family together as they face the difficult challenges of end of life.

(Home Health) Physical & Occupational Therapists and Aides

As a member of our therapy team, either as a physical or occupational therapist, your work will be an essential element of the care plan, designed to help our patients return to their highest level of function. Help patients to improve movement, manage pain or achieve independence with everyday activities.

Our therapy aides work at the direction and supervision of our physical and occupational therapists and assist in helping patients who are recovering from surgeries or injuries manage pain and regain their mobility. In this role, you will assist in making an impact on the patient’s activities of daily living, identify factors affecting their physical and psychosocial comfort and attend to these needs.

(Hospice) Chaplain

Spiritual support can bring peace and acceptance to patients and families facing the end of life. Our hospice chaplains embrace this role, being a “sounding board” for families struggling during this difficult time. As a chaplain on our hospice team, your compassion and guidance will help families of all faith backgrounds.
