MAJOR RESPONSIBILITIES

  • Provides care for patients enrolled in an IDPH approved Mobile Integrated Healthcare Program.  Collaborates with nursing and case management staff for pre-planning and patient rounding on identified patients.  Completes home safety evaluation as required per the MIHP.  Coordinates compliance / wellness check with patient / caregiver.
  • Review of medication compliance / sorting during patient visits.  Evaluate caregiver involvement.  Assess for referral to other outpatient care providers (i.e., home health, PT, etc.).  Provides ongoing patient education.
  • Patient care interventions as dictated in the discharge care plan as well as ongoing interaction with healthcare providers.  Coordination of care in reporting to discharge planner, home health agency or primary care provider as required.
  • Compiles community resources available for patients (i.e. pharmacy, mental health, transportation, utilities, food, etc.).
  • Completes required documentation.  Reviews community assessment tool in conjunction with manager.  Ensures completion of Primary Care Provider Collaborative Agreement.  Ensures completion of post discharge adult patient enrollment form.
  • Completes recommended resident safety assessment form, waiver / release.  Completes patient assessment documentation.  Reviews patients’ medications as ordered on discharge.  Complete patient care report.  Complete all required QA / QI documentation as stipulated by IDPH and Local EMS system.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.  Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.  Age-specific information is developed further in the departmental job standards.

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIRED

 

License/Registration/Certification:  EMT-Paramedic and BLS for Healthcare Providers

 

Level of Education:  High School diploma or GED

 

Years of Experience:  4-5 years of clinical / field experience at the Paramedic level 

 

 

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES (KSA)

  • Competency in EMS procedures and knowledge as defined by the EMS system.
  • Competent patient assessment skills.
  • Ability to work collaboratively as a member of the healthcare team.
  • Good communication and social skills.
  • Empathy.

 

 

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  • This position requires travel, therefore, will be exposed to weather and road conditions
  • Operates all equipment necessary to perform the job.
  • Exposed to a normal office environment.
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other teammate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
