8:00-4:30, 8:30-5 rotation Saturday 6:30-3pm- Onsite
Major Responsibilities:
- Respond to health record requests from attorneys, insurance companies, physicians, patients, as well as subpoenas.
- Respond to subpoenas, attorney’s, physician’s, internal departments and patient requests for health records and Radiology films or CDs. Also responding to all Occupational & Employee Health subopenas, doing research, making corrections, speaking with Attorneys and their staff ensuring the accuracy of all subopenas. Entering all notes into Systoc system. Prepping and scanning all subopenas into On-Base system for future reference.
- Performs the Functions of a Radiology File Clerk
- Review record logs of patient records stored at off-site facility and on microfilm for aging to ensure proper retention and timely destruction of records based on regulatory rules.
- Answer telephone inquiries from patient’s physicians or their office staff for all health records, films and reports courteously and professionally with a primary focus on providing superior customer service.
- Compile and process daily patient registration, clinical information and health records release forms for both AMG High Tech, Palos and AMG High Tech, Tinley Park into the OnBase record storage system, also entering information into Systoc system.
