Schedule Details/Additional Information:
The schedule is 4/10-hour shifts from 8a-6:30p, rotating weekends which is about once a month, and rotating holidays. We are currently a hybrid role and work one day a week in the Oak Brook office. On call rotation.
We are the first line of contact with family and patients and set the tone for their experience.
Major Responsibilities:
- Responsibility for the coordination of all hospice referrals and comply with state and federal regulations. Serves as the internal point person and processing agent for all new hospice referrals.
- Processes referrals for all programs for Hospice. Verification Specialist and Patient Account Specialists.
- Responds to all inquiry calls regarding Hospice care.
- Identifies risk management situations and reports to supervisor immediately.
- Interprets company services to referral sources and individuals seeking information
- Maintains a knowledge of Medicare, Medicaid, HMO procedures and Managed Care guidelines.)Explores reimbursement in conjunction with Insurance Verification specialist / Patient Account Specialist
- Provision of Quality- Maintains collaborative relationship with referral sources. Serves as the hospice point person for referral sources to coordinate the referral to admission process.
- Responds courteously and positively to client requests and concerns and assures appropriate resolution.
- Maintains open communication with referral source and ensures that they are informed of the most recent status of the referral.
- Performs other duties as assigned; i.e., on call rotation
- Coordination of referral process for Advocate Hospice.
- Responsible for cross referencing patient name in EMR system and basic data entry
- Responsible for the coordination of scheduling the initial appointment with patient/family and the hospice admission team member, then updating the referral source with the time and date of the appointment.
- Coordination of required documents prior to admission. This includes ensuring that there is a physician order to evaluate and a recent H&P (in the last 12 months) and ensuring that the H&P is a part of the EMR.
- Responsible for following up with previous referrals that are in a pending status to schedule initial or follow up admission appointments.
- Responsible for the coordination of admission consent documents, including their tracking and scanning
- Follow up with previous referrals to schedule appointments.
- Also responsible for tracking and scanning in consents for previous admissions.

Education/Experience Required:
- HS Diploma or equivalent. 3 years of hospice intake experience
Knowledge, Skills & Abilities Required:
- Very good communication skills.
- Knowledge of Medicare rules and regulations.
- Strong Customer Service and communication skills
- Working knowledge of managed care environment
- Ability to work in a very fast paced and demanding environment
- Ability to multi-task and prioritize patient scheduling
- Teamwork
Physical Requirements and Working Conditions:
- Ability to work in a fast paced environment with established time constraints.
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
Additional Information:
The schedule is 4/10-hour shifts from 8a-6:30p, rotating weekends which is about once a month, and rotating holidays. We are currently a hybrid role and work one day in the Oak Brook office. Training schedule would be 8a-4:30p or 8:30-5 and would be required to be in person training Monday- Friday.
