Schedule Details/Additional Information:
Monday – Friday 8am to 430 PM;1 summer and 1 winter holiday oncall per year.

Job Description Summary
The Home Health Aide is a vital part of the interdisciplinary team at Advocate Hospice Services, working to achieve the vision of the organization by delivering cost-effective, patient-centered care. As an essential member of the team, you will contribute to the overall success of our service by meeting performance expectations and participating in achieving team goals.
Purpose of the Position
This position is designed to deliver results-oriented, customer-focused care. You will be responsible for providing competent care to assigned patients, contributing to effective patient outcomes, and ensuring your responsibilities are consistently met.
Schedule Details
- Monday – Friday: 8:00 AM – 4:30 PM
- On-Call Requirements: 1 summer and 1 winter holiday on-call per year
Position Requirements
- High School Education or equivalent.
- One year of Nurse’s Aide experience or enrollment in an RN program that meets state requirements.
- Certified Nursing Assistant (CNA) in the state of Illinois.
- Ability to assist with ambulation and transfer patients weighing up to 200 pounds.
- Must be able to travel to patient homes in all weather conditions and across different types of roads, including expressways.
- Ability to lift up to 35 lbs without assistance; for heavier lifts, patient handling equipment is used with help from at least one other associate.
- Ability to recognize and understand the needs and behaviors of various age groups, including children, adolescents, and older adults.
- Effective written and verbal communication skills in the English language.
Key Responsibilities
- Deliver high-quality patient care to enhance patient comfort and overall well-being.
- Assist with daily living activities such as bathing, dressing, and grooming.
- Support patients with mobility needs including transferring, turning, and ambulation.
- Follow care plans and document patient progress to ensure accurate and timely information.
- Collaborate with the care team to meet patient needs and ensure the best possible outcomes.
- Participate in Continuous Quality Improvement (CQI) processes and adhere to all regulatory requirements.
