The LPN assists in planning, implementing and evaluating nursing care under the supervision of an RN. The LPN recognizes the needs and behaviors of patients and provides care in a manner that is age appropriate. The LPN strives for quality patient care which assures the patient is treated in a holistic manner with dignity and respect.
Major Responsibilities:
Delivers patient care under the guidance of a professional registered nurse and according to the patient care plan.
1)Provides selected nursing procedures and care based on nursing plans of care
2)Identifies crisis and/or problems while performing holistic care and manages appropriately.
3)Performs selected therapeutic measures prescribed by doctors.
4)Learns, supports, promotes and complies with the policies and procedures, the standards and regulations of the nursing practice act, the hospital and all regulatory bodies.
5)Utilizes basic safety measures and principles.
Administer approved medications, treatment and IV therapy per Nursing Protocols.
1)Administers selected medications as directed in the medication administration manual.
2)Recognizes action, side effects and compatibility of prescribed medications in order to provide safe administration.
Reinforces patient teaching plans and discharge planning in accordance with the plan of care.
1)Reinforce patient and family teaching as provided by the physician and the RN.
2)Implements discharge planning as soon as possible by documenting patient generated information in relations to home environment, physical, spiritual, and psychological concerns.
Collaborates with health care team to implement medical plans of care and implement nursing care plans.
1)Assists in the implementation of the nursing care plan reflecting identified problems and needs under the direction of the registered nurse.
2)Contributes to the care plan for assigned patients and updates the plan in response to the patient’s changing needs in collaboration with the RN.
3)Completes all patient orders and interventions, demonstrates responsibility for clinical decision making, and assumes patient advocacy role, protecting patient rights.
Documents care, medications, and observations made during the care of patients.
1)Documents all changes in patient condition with actions taken and notifies appropriate authority.
Participates in professional development by attending and participating in in-services and continuing education programs.
1)Participates in in-services, continuing education programs and/or workshops to increase knowledge and skill in order to maintain nursing competence and to facilitate personal growth.
2)Demonstrates teaching strategies with patients, families and other health care members.
Utilizes communication skills, promotes multidisciplinary coordination, and cooperative working conditions.
1)Uses verbal and non-verbal communication skills to identify and reduce patient/family anxiety.
2)Collaborates with other heath team members to coordinate patient care, to create a cooperative and safe working environment, and to ensure patient privacy and confidentiality.
Demonstrates commitment to the nursing department and organizational goals, and to the standards and practices of all regulatory agencies.
1)Participates in unit meetings and activities, keeps all nursing knowledge and practices current.
2)Attends to personal work schedule in a consistent and professional manner according to established departmental guidelines.
Licensure:
Nurse, Licensed Practical (LPN)
Education/Experience Required:
• Completion of an accredited or approved nursing program, or • Receipt of a certificate of completion of pre-licensure requirements
Knowledge, Skills & Abilities Required:
Clinically appropriate skills. Ability to cooperate and work with others. Good communications skills.
• Current Licensed Practical Nurse (LPN) license issued by the state in which the team member practices
Physical Requirements and Working Conditions:
-Ability to work weekends, holidays and different shifts in order to accommodate staffing needs. -Ability to lift and/or move patients up to 100lbs may be requested. -Ability to work under stressful conditions and in difficult situations. -May be exposed to hazardous materials and life-threatening diseases. -Ability to perform lifting/transfer activities related to patients. -Ability to work rotating shifts any day of the week.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
