Major Responsibilities:
- Reviews, evaluates, and consults with patients and other health care providers on issues relating to drug related problems and optimal drug therapy. Reviews and accurately interprets physician orders.
- Checks to prevent allergic or adverse drug reactions and reports them through appropriate channels.
- Reviews and interprets medication histories in order to make recommendations regarding drug therapy, drug dosing, or the process of drug delivery to other health care providers.
- Reviews and evaluates drug related information to make recommendations regarding the most effective, cost-effective drug therapy.
- Clarifies the patient's understanding of their therapy to ensure the best possible outcome. Assures that the medication needs of patients are met throughout the continuum of care.
- Prepares and checks medication doses in a manner consistent with good pharmaceutical practice, including using appropriate compounding technique, packaging, and labeling of substance.
- Distributes drugs in accordance with Wisconsin Administrative Code relating to the practice of pharmacy, providing appropriate adjunctive information to the patient or intermediary.
- Oversees the processing of central fill claims and maintains drug files for billing and system-wide inventory control purposes. Responsible for filling, maintaining, troubleshooting, and reporting functions of central fill database systems and programs.
- Provides work direction and guidance for technicians in performing technical pharmacy functions, including checking the technician's work to maintain high standards of pharmaceutical care. Supports and participates in department performance improvement activities, department committees, and shared governance activities.
- Serves as a clinical and information resource for all central fill process related questions throughout the system. Provides input into establishing and implementing written policies and procedures, and workflow processes for central fill pharmacy operations.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Pharmacist license issued by the state in which the team member practices.
Education Required:
- Bachelor's Degree in Pharmacy.
Experience Required:
- No experience required.
Knowledge, Skills & Abilities Required:
- Excellent communication (written and verbal) and interpersonal skills.
- Strong customer service and organizational skills.
- Demonstrated ability to work effectively with providers, staff and patients.
- Must be detail-oriented with the ability to handle multiple demands simultaneously.
- Intermediate computer skills including the ability to navigate through a database and run reports.
Physical Requirements and Working Conditions:
- Exposed to a normal clinic office environment.
- Will occasionally lift up to 25 lbs. without assistance.
- Must be able to stand up majority of the workday.
- Must have functional speech, vision, and hearing.
- Will be exposed to chemical hazards; therefore, must have the ability to wear protective clothing as needed.
- Operates all equipment necessary to perform the job.
