Schedule Details/Additional Information:
1st Shift, Monday thru Friday, no evenings weekends or call.
Major Responsibilities:
Monitors and maintains an optimal level of productivity through the management of day-to-day program/department operations. Develops and recommends operating and capital budgets and controls expenditures within approved budget objectives.
Develops, implements, and updates policies and procedures to ensure a safe and efficient flow of service activities. Develops documentation around processes and trains staff on ongoing changes.
As appropriate, holds staff accountable for daily operational detail and for informing team members of important and critical issues and concerns.
Aligns social work programs and services with Community Health strategic goals and priorities and in support of the Advocate National Center for Health Equity.
Advances social work practices through performance and quality-based outcomes.
Participates in multidisciplinary, cross-functional efforts to ensure high-quality, cost-effective systems and processes to support best practice.
Works collaboratively with key internal and external stakeholders.
Promotes a Health Equity approach across all programs while customizing care for unique patient populations and health environments. Promotes patient care services that are inclusive and grounded in a cultural humility framework.
Designs and directs the work of the team(s). Selects, coaches, and develops team(s). Sets clear expectations to inspire and motivate the team. Manages performance by recognizing achievement, providing feedback, and administering progressive discipline when necessary.
May develop and/or recommend operating and capital budgets and controls expenditures within approved budget objectives.
Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that people actions, and the actions of employees supervised, comply with the policies, regulations, and laws applicable to the organization's business.
Licensure, Registration, and/or Certification Required:
Certified Advance Practice Social Worker or Licensed Clinical Social Worker.
Education Required:
Master's Degree in Social Work (MSW)
Experience Required:
3-5 years of demonstrated experience providing social work services within a health care setting. Demonstrated experience working with diverse populations. Demonstrated experience leading teams and/or team-based projects and initiatives.
Knowledge, Skills & Abilities Required:
Knowledge of the social drivers of health.
Demonstrated leadership skills and abilities, including organization, prioritization, decision-making, delegation, team building, customer service, and conflict resolution.
Excellent communication and interpersonal skills.
Must have the ability to work and establish relationships with a variety of patients, staff and leaders throughout the organization and the community.
Excellent organizational, analytical, and problem-solving skills.
Must be detail-oriented and have the ability to manage multiple tasks.
Demonstrated creativity, sensitivity, and knowledge of community resources.
Demonstrated ability to work collaboratively using a strengths-based and solution-orientated approach.
Intermediate experience with computers, data entry, and database software required.
Exceptional human relations, organizational, communication, and facilitation skills, with demonstrated ability to manage multiple areas and projects.
Physical Requirements and Working Conditions:
This position requires travel, therefore, will be exposed to weather and road conditions.
Operates all equipment necessary to perform the job.
Remote-based work from home and office.
