Schedule Details/Additional Information:
Monday – Friday from 9am to 5pm. Some Weekends as needed.
MAJOR RESPONSIBILITIES
- Supports operations including scheduling, consent documentation and school partner needs in the assigned service areas.
- Serves as a community liaison providing information about mobile health services to new partners, coordinates program logistics with community partners.
- Supports data collection for mobile health program for patient monitoring and reporting to organization and funders.
- Assists leadership in appropriate community partnership development.
- Plans participation in strategic community events to increase visibility, accessibility, health promotion and community partnerships.
- Determines appropriate level of participation in community events and activities.
- Works closely with manager on recruitment, scheduling, and logistics of special event team members.
- Partners in the development of community education/lectures, develops/updates existing exhibits and resources for adults and children.
- Participates in appropriate health and community-based collaboratives.
- Supports team at time in the Mobile Unit with onsite events and patient registration data. Supports data collection for mobile health program for patient monitoring and reporting to organization and funders. Includes annual and ad-hoc reports supporting the impact of service line.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
Level of Education: Bachelor’s Degree in Community Health or related field.
Years of Experience: 3 years Community Health, Public Health, outreach, social work, communications, education, or similar field.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
- This position requires travel, therefore, will be exposed to weather and road conditions.
- Operates all equipment necessary to perform the job.
- Exposed to a normal office environment.
- Ability to lift, transport necessary equipment, displays throughout hospital and community. Ability to maneuver in tight spaces (i.e. automobiles to install car seats) Ability to cooperate and work well with others.
- Ability to lift up to 35 lbs. without assistance.
