Education/Experience Required:
• MSW, LSW

Major Responsibilities:
Completion of an assessment/history on all referred patients within appropriate time frames.
Coordination of discharge planning, based on assessment of patient and family needs, preferences and available resource

Provision of counseling and supportive services for patients and families in crisis and/or suffering from acute emotional distress.
Participation in the interdisciplinary team, sharing expertise to assist in the diagnostic and treatment planning process.

Maintenance of professional standards and responsibilities for his/her own professional practice according to accreditation, hospital, system, state and NASW Standards and Code of Ethics.

Cultivation and maintenance of relationships with community resources and the community at large.

Knowledge, Skills & Abilities Required:

• LSW • Licensure in social work requires 30 contact hours of continuing education every 2 years
• Has demonstrated time management, organization and documentation skills • Ability to understand and lead group process Demonstrated understanding of medical terminology, developmental stages and psychosocial treatment • Demonstrated ability to work in an interdisciplinary team • Therapeutic communication skills

• Ability to handle crisis situations, and effectively • Weekend, holiday and evening coverage • Resiliency and flexibility to effectively work within a constantly changing health-care environment • Demonstrated problem-solving ability • Fast paced work environment with established time constraints • Ability to handle multiple tasks and complete work within short timeframes • May be exposed to hazardous materials and life threatening diseases • Knowledge of universal precautions • Works within and supports a shared governance model • Is customer and service focused • Ability to flex hours and days in order to meet patient care and family needs
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
