Schedule Details/Additional Information:
This HHA will support our entire West Team – Crystal lake to Elgin (north to south) and Algonquin to Lake In The Hills (east to west) and surrounding towns

Advocate Aurora Health is offing a $2500 sign on bonus for Home Health Aides. 

Position Requirements:
High school education or equivalent.
One year nurse's aide experience.
RN nursing students that meet state requirements.
Recognize the needs and behaviors of a variety of age groups of patients treated, understand child growth and development, i.e., understand adolescent behavior and look for signs of normal aging.
Ability to communicate with patientsand staff.
Must be certified as a nursing assistant in the State of Illinois. Must be able to travel to all of service office boundaries in all extremes of weather on all types of road including expressways.
Must be capable of moving/lifting patients. Ability to perform very heavy lifting (lifting up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly.
Required to make visits to private residences which may not be accessible to people with disabilities.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Purpose:
The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required.

Provide quality patient care in accordance with Advocate/South Suburban hospital policies and procedures.

Perform various patient care activities and non~professional services to promote patient's comfort while promoting the patient's optimal level of self~care.
