Schedule Details/Additional Information:
Monday – Friday, 8:30a-5:00p,JOB SUMMARY: Serves as the social worker for GUIDE (Guiding an Improved Dementia Experience) program to connect patients and caregivers living with Alzheimer’s disease and related dementia (ADRD) with necessarily medical and social resources. Provides social work services, complex case management, and care coordination to patients and their families who are seen in the GUIDE program with collaboration with existing primary care providers, neurologists, and other community partners. Assess need for community resources and barriers to healthcare. Make referrals as appropriate. Collaborate with members of the multi-disciplinary team to yield positive patient and family outcomes. In conjunction with the multi-disciplinary team, assists with the development of a comprehensive care plan to meet ongoing health and mental health needs related to dementia.License/Registration/Certification:   Licensed Clinical Social Work Illinois (LCSW), and Accredited Case Manager preferred (ACM-SW) preferredIssued by (Governing Body):  Click here to enter text.Level of Education:  Master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE) requiredYears of Experience:   Three years experience in social work   :   Two years experience in care of targeted patient population in a broad range of clinical situations/settings.    

  • Major Responsibilities: CANDIDATE MUST HAVE MSW EXPERIENCE IN HOSPITAL SETTING
    • Timely and complete documentation to meet licensure and third party payor requirements
    • Provision of in-services to assigned units and attendance of educational seminars as appropriate.
    • Contributes to Continuous Process Improvement activities

    Licensure:

    • Social Worker, Clinical LCSW

    Education/Experience Required:

    • MSW from accredited school of Social Work and field experience within a hospital setting. The candidate must recognize age specific needs and behaviors. Must adhere to NASW code of ethics and standards.

    Knowledge, Skills & Abilities Required:

    • Demonstrates competence in counseling and discharge planning.
    • MSW (Licensed Social Worker Masters) from accredited school of Social Work and field experience.

    Physical Requirements and Working Conditions:

    • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
