Schedule Details/Additional Information:
40 hours per pay period. Will share position with existing Hospital School Liaison.

MAJOR RESPONSIBILITIES

  • Responsible for communicating with individual patients/patients families and schools to assess academic needs relative to the patient’s health, school participation, and performance.   Responsible for collaborating with individual patients/patient’s families, medical team, and school to identify specific structures, strategies, accommodations, and modifications to appropriately support the patient’s curricular participation and performance.
  • Gather pertinent patient’s health information from multiple sources including the patient, patient’s family, medical team, care team, medical records, etc. to assess the impact on the patient’s active and consistent participation in academic requirements and to discern appropriate structures, strategies, accommodations, and modifications relative to the patient’s individual health needs.
  • Facilitate communication between the patient, patient’s family, medical team, care team, and school so all parties understand the impact of the health needs on school performance and appropriate school supports.
  • Collaborate with school districts, medical staff, and patient family members to create 504 Plans, IEP plans, and set up Homebound Tutoring as appropriate.  Responsible for collaborating and participating in an interdisciplinary plan of care to facilitate school supports aligned to the context of the patient’s familial structure, health, and psychosocial development.
  • Responsible for personal and professional growth.  Seeks out educational opportunities, stays abreast of current standards of care, and stays abreast of special education codes. 
  • Maintains up to date knowledge of the state of Illinois board of education legislation and standards.  Educate and assist families in obtaining educational resources within their rights.
  • In addition to supporting school age children, this position often works with college-age young adults to facilitate supports at the collegiate level as well.
  • Coordinates, plans, and implements events with patients and families to ensure typical development and enhance coping while receiving medical treatment.
  • Partners with Public Affairs and Marketing and Advocate Charitable Foundation to coordinate and implement special events with donors and community partners in order to develop, nurture and maintain relationships. Maintain all in-kind donations and utilize restricted funding per donor intent.  Manages delivery and storage of all in-kind donations, toys, and supplies.

SUPERVISORY/MANAGEMENT RESPONSIBILITIES

Has oversight for public school teachers that are assigned to Advocate Children’s Hospital.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED

License/Registration/Certification:  Type 09 or 10 Illinois Teaching Certification

Issued by (Governing Body):  Illinois State Board of Education

Level of Education:  Bachelor’s degree in Education or related field

Years of Experience:   2 years teaching experience with demonstrated ability to develop individual education plans.

Describe Type Experience:  Student teaching experience Kindergarten through 12th grade.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES (KSA)

  • Strong PC skills and understanding of current technologies utilized for education in schools.
  • Proven leadership skills, ability to prioritize in a fast paced and high demand setting, and strong organizational skills.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served (typically 5-21 years). 
  • Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific and/or developmentally appropriate needs.
  • Experience and comfort working with hospitalized children, patients diagnosed with mental illness, developmental delay, and/or who displays challenging behaviors in individual and group contexts.
  • Strong interpersonal and communication skills as demonstrated through establishing and maintaining positive relationships with patients, parents, team members, other teachers, administrators and students.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  • This position may require travel to patients schools for IEP/504 meetings, therefore, will be exposed to weather and road conditions.
  • Operates all equipment necessary to perform the job.
  • Exposed to a normal office environment.
  • May be required to lift up to 25 pounds without assistance and over 25 pounds with assistance.
  • Contact with patients with infectious diseases, which requires special protective equipment, training, and adherence to prescribed rules and procedures to prevent and spread infection.
  • Requires direct contact with patients, families, AAH team members in all departments, intern students, volunteers, representatives from community groups and agencies, professionals from other health care institutions, and professional organizations.
  • Must be able to sit, stand, walk, squat, bend, and reach above shoulders and twist.
