Schedule Details/Additional Information:
Home Health Occupational Therapist Case Manager to support the Marinette patient service market. Normal office operating hours are 8:00am to 4:30pm. Part time position allows for some flexibility; final usual schedule to be determined upon hire. Position will require a weekend rotation, usually once every 5th to 6th weekend.
💼 What You’ll Do
- Evaluate patients’ physical, cognitive, and social abilities to tailor personalized care plans
- Teach adaptive techniques for daily living, mobility, and home safety
- Collaborate with physicians and care teams to ensure optimal outcomes
- Recommend and train on adaptive equipment
- Supervise Occupational Therapy Assistants (OTAs) as needed
- Maintain accurate clinical documentation and manage your caseload
🎓 What You Bring
- Bachelor’s Degree in Occupational Therapy
- Wisconsin Occupational Therapist License
- BLS Certification (or willingness to obtain within 6 months)
- Valid Driver’s License
- 3+ years of experience in acute hospital, home health, or similar settings
- Strong clinical judgment, communication, and organizational skills
