Schedule Details/Additional Information:
36 hours per week. (7:30 – 4:00pm or 8:00 – 4:30pm or 8:30 – 5:00pm). May have to work occassional holiday or weekend – based on department needs.

Major Responsibilities:

  • Reviews patient records using appropriate software (Epic) to identify patients' plan of care before rounding. Performs clinical patient rounding and promptly communicates observations, concerns, and needs of patient/family to care team.
  • Documents and tracks findings of all patient rounds within the Cipher Rounding Tool to track outcomes for identifying trends and opportunities for improvement and provides weekly updates to leader.
  • Provides the highest level of patient service by problem solving, documenting patient concerns, and identifying appropriate actions for resolutions prior to patient being discharged.
  • Acts as a patient advocate to any potential patient concerns and collaborates with inpatient medical teams to escalate identified concerns to ensure a safe discharge.
  • Provides support in addressing and communicating complex cases with care team that require higher level management due to acuity, lack of adequate support, poor health literacy, or social determinates of health.
  • Facilitates the patient and family’s right to receive quality care by demonstrating a commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to meet those needs and achieve positive patient outcomes.
  • Collaborates with Care Transition RNs (Registered Nurse) by sharing/flagging readmitted, or high-risk patients for a call after discharge.
  • Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care.
  • Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations, and laws applicable to the organization's business.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices.

Education Required:

  • Completion of an accredited or approved program in Nursing.

Experience Required:

  • No experience required.

Knowledge, Skills & Abilities Required:

  • Working knowledge of the components of quality and acute patient care needs.
  • Excellent communication and interpersonal skills; ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff, and physicians.
  • Proficient computer skills including keyboarding, navigation within a windows operating system, and use of electronic mail with exposure to electronic medical records systems.
  • Demonstrates customer service skills that support a positive patient experience.

Physical Requirements and Working Conditions:

  • Ability to work weekends, holidays, and different shifts to accommodate patient needs. Ability to respond quickly to patient needs and work at fast pace. Ability to stand for up to eight hours.
  • Work environment contains potential for exposure to blood and body fluids or other unpleasant fumes or hazards; therefore, personal protective equipment must be worn as necessary.
  • Operates all equipment necessary to perform the job.
