Position Summary
Explore an exciting opportunity with AHWFB’s Hospital Medicine section as we eagerly welcome a skilled Nurse Practitioner (AGACNP) or Physician Assistant to join our dynamic Hospital at Home team. This role is integral to the Hospital at Home (HaH) program, delivering virtual acute care supported by paramedics, nurse navigators, and ancillary staff. The Advanced Practice Clinician (APC) will collaborate closely with Hospital at Home physicians to ensure high-quality, patient-centered care.

Responsibilities
Deliver virtual medical care and develop patient care plans using sound clinical judgment. Collaborate with physicians and interdisciplinary teams to ensure coordinated care. Diagnose and treat internal medical conditions via telehealth platforms. Conduct virtual patient assessments and order appropriate diagnostic tests. Interpret test results and prescribe treatments, including medications and therapies. Provide patient education and participate in goal setting. Perform procedures as per departmental guidelines. Uphold AHWFB’s mission, values, and cultural commitments. Attend regular team meetings and contribute to continuous program improvement. Maintain timely and accurate medical documentation.
Qualifications
- For Physician Assistants:
- Graduate of an accredited PA program; Master’s preferred.
- Current NC Medical Board license and NCCPA certification.
- Minimum 2 years of hospitalist experience; outpatient and/or telehealth experience a plus.
- Active DEA registration.
- For Nurse Practitioners:
- Master’s degree or higher from an accredited AGACNP program.
- Acute care certification
- Current NC RN and NP licenses
EDUCATION/EXPERIENCE:
- Graduation from an accredited Physician Assistant Program, Master’s degree preferred
LICENSURE, CERTIFICATION, and/or REGISTRATION:
- Current applicable state Medical Board Medical License
- Certified by NCCPA
ESSENTIAL FUNCTIONS:
- Using medical decision-making skills to determine appropriate patient care plan.
- Collaborates with medical team when needed for effective team-based care.
- Order and interpret laboratory studies and diagnostic procedures. Explains the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
