Major Responsibilities:

The RN in Advocate Health Care’s Home Health division plays a vital role in delivering high-quality nursing care to homebound patients across Illinois. This position involves managing patient care from assessment through evaluation, fostering strong therapeutic relationships with patients and caregivers, and coordinating medical needs with a multidisciplinary team. The nurse helps patients transition from hospital to home, promoting independence and preventing rehospitalization. Responsibilities include documenting care at the point of service, educating patients and families, and supporting the professional development of peers. The RN also demonstrates leadership, clinical expertise, and collaboration with other healthcare professionals to ensure holistic, cost-effective care. Visits are made to private residences, which may present accessibility challenges, and the role requires physical capability to assist with patient mobility.

They are responsible for patients safely to and from imaging areas, preparing exam rooms, and assisting technologists with procedures. Additionally, they manage and document patient information accurately, act as a liaison between physicians, technologists, and patients to ensure smooth communication and efficient workflow, and maintain imaging equipment while organizing necessary supplies.

• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) must be obtained within 6 months unless determined otherwise by the department leader

Education Required:

• High School Diploma or equivalent required

• We are willing to take newly graduated imaging students from accredited imaging programs
