Tuesday – Saturday – 8:00-4:30

Major Responsibilities:
Coordination of “Important Message from Medicare” (IM) correspondence.
4)Obtains signatures from Medicare Beneficiaries or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.

5)Mails out appropriate letters to emergency contacts when the Medicare Beneficiary is unable to take receipt of the acknowledgement or when directed by the Care Manager of Social Work Staff.

6)Monitors all department voicemail lines and follows up appropriately.

1)Delivers the IM letter/Code 44 letter to the Medicare Beneficiary or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.

2)Documents all activities related to the IM/Code 44 in Care Connection.

3)Provides clerical support to the Care Managers and Social Work Staff during the appeals process for a Medicare Beneficiary.

Coordination of discharge planning.
1)Provides required documents to vendors in a timely manner, faxing appropriate information from the patient’s medical record when necessary.

2)Assists with facility bed availability and updates the Care Manager or Social Worker of status of facility as needed until patient discharge when necessary.

3)Identifies contracted vendors for discharge relating to home health care, nursing home placements, durable medical equipment, LTACHs, Hospice etc. Makes the appropriate referrals and communicates arrangements to the Care Manager or Social Worker, performing inquiries about benefits and eligibility when necessary.

4)Monitors all department voicemail lines including the social work voicemail and follows up appropriately.

Compilation of department statistics as requested.
1)Updates hospital systems related to discharge.

2)Provides reports as needed as directed by the CM Leadership.

Provide Clerical support to the department.
1)Order supplies for the department.

2)Take meeting minutes for departmental as well as ad hoc meetings when requested.

Education/Experience Required:
• H.S. diploma or equivalent • 2-3 years clerical experience in a health care setting

Knowledge, Skills & Abilities Required:
• Ability to use Microsoft Office • Ability to handle difficult calls and maintain professional conduct • Good communication skills
