Major Responsibilities:

  • Provides strategic input, develops and implements the Home Health program within the Continuing Health division. Monitor business and marketing plans to stimulate growth and increase capture within the Advocate Health Midwest Region. Assure that the program is integrated operationally and aligned with the organizational goals and strategies. Monitors and evaluates program performance and recommends and helps implement appropriate changes to leadership and stakeholders to achieve optimal results.
  • Accountable for developing strategic and annual operating plans for home health (multi -state). Assuring that the program is in alignment with the continuing health division. Integrates national best practices and benchmarks into the program to ensure optimal outcomes for the healthcare system and consistent operational delivery of services.
  • Accountable for all communication and data reporting to Executive Leadership Team, senior leaders, care management councils, medical staff leadership, and all other key stakeholders. Maintains strategic relationships with executive leadership in business development and sales to ensure the program is meeting the needs of our patients and families.
  • Provides leadership in planning, developing and providing high quality, cost effective services to the patients and families.
  • Leads in setting performance management goals, operating plans, dashboards, and outcome tracking tools to ensure performance is monitored, on target and achieving intended outcomes. Accountable for the system wide integrated care management dashboard and outcome tracking tools to ensure performance is monitored and on target to achieve intended outcomes. Provides evidence-based framework and consultation regarding best practices with specific emphasis on system implementation and workflow design.
  • Develops collaborative and cooperative relationships with system leadership, skilled provider administrators, customers, physicians, community organizations and entities of the Advocate Health Midwest Region .
  • Plans and develops budgets that allows for growth and innovation. Monitors revenue and expenses for programs to assure adherence to budget.
  • Implements and supports new technology within programs including telehealth and virtual monitoring.
  • Implements and assesses clinical quality and patient experience initiatives to accomplish targets and ensures the highest possible delivery of service to patients, families, physicians and team members relative to their personal expectations.

Licensure, Registration, and/or Certification Required:

  • None Required.

Education Required:

  • Master’s degree in health care administration or related field.

Experience Required:

  • Typically requires 10 years of experience in progressive health care leadership experience in driving operational and/or strategic programs. Experience in a home health and post acute setting is desired along with settings within a complex integrated health care system. Includes 3 years of management experience in staff and budgets.

Knowledge, Skills & Abilities Required:

  • Demonstrated ability to effectively manage a large team in a professional manner.

  • Thorough understanding of home health regulations, reimbursement and care requirements.

Physical Requirements and Working Conditions:

  • This position requires frequent travel, so there is exposure to road and weather hazards.
